Form VO-1-001, the Vessel Operator Enrollment Application (Version 001)

1. Name and address of the Vessel Operator.
2. Point of Contact. (see "Definitions" below)

3. Principal. (See "Definitions"below)

4. Areas of Operation. In the space provided, list the main geographic areas of operation for the vessel operator.
5. Related Entities, Partners, and Previous Names. Pertaining to vessel operation, employing or training DPOs, or related purposes, check the below that apply.


6. Signers of Sea Service Letter (Form DPO-1-CV). In the spaces below, list the name(s) of the individual(s) that will be responsible for signing Form DPO-1-CV, the Training and Certification Activities Confirmation Letter (Current Version).(Attach additional sheets if necessary.)










7. DP Related Documents. Attach to this form examples or detailed summaries of the operations manuals and/or decision support tools ASOGs, CAM/TAM, standing orders, and client instructions utilized aboard the vessels in the Vessel Operator's fleet. The document(s) included should be indicative of the document(s) utilized across the Vessel Operator's entire fleet.
8. Assessment Instructions. Attach a copy of the Vessel Operator's instructions provided to their mariners for how OSVDPA assessments shall be conducted and include the name of the attachment in the space provided below. As specified in OSVDPA MPP-1-CV, the OSVDPA Manual of Policies and Procedures (Current Version), these internal policies must (at a minimum) specify that the Vessel Operator will abide by the OSVDPA policies and procedures regarding assessments and require its employees do as well, how the Vessel Operator shall protect the confidentiality of OSVDPA assessments, and how the Vessel Operator will comply with the annual reporting requirements.
9. Use of Qualified on Board Assessors (QOBAs). Describe how the Vessel Operator shall utilize its personnel as Qualified on Board Assessors (QOBAs), e.g., does the Vessel Operator intend to utilize a limited number of QOBAs or will the Vessel Operator attempt to have all of its captains capable of conducting assessments?
10. Letterhead. Attach to this form a copy of the Vessel Operator's official letterhead and include the name of the attachment in the space provided below.
11. Vessel Information. This section is optional and is included as a service by the OSVDPA to ensure that DPOs from the Vessel Operator have their certificate requests evaluated in an expedited manner. Vessel Operators can include as little or as much information as they see fit in this section. Please use Form CVA-1-CV, the Classed Vessel Application (Current Version) to submit additional vessels to the OSVDPA Database.











12. Sea Service Confirmation Letter Approval.

As stated in Section 13 of OSVDPA MPP-1-CV, Enrolled Vessel Operators are allowed to substitute their own sea service confirmation letter for completion of Form DPO-1-CV, the Training and Certification Activities Confirmation Letter, provided these letters follow a template approved by the OSVDPA as meeting minimum standards.

Vessel Operators that seek to utilize their own sea service letter to confirm a DPO or Prospective DPO's Sea Time shall attach a template of this letter to this form.

Note: This section is optional and is included to provide Vessel Operators another way to confirm the Sea Time recorded by DPOs and Prospective DPOs.

13. Disclaimer, Signature and Relationship Agreement.

The below must be signed by the Principal (see "Definitions and Instructions" below).

14. Definitions.

Auditor. An individual approved by the OSVDPA as capable of systematically and independently obtaining and objectively evaluating evidence for the purposes of determining if the asset or entity meets the criteria developed by the OSVDPA.

Enrollment. The process of the OSVDPA approving a Vessel Operator in order for the OSVDPA to accept Phase 4 Assessments, Phase 5 Assessments, and Re-validation Assessments conducted by the Vessel Operator on its vessels.

Participant Rights and Responsibilities Acknowledgement. This form must be signed by certain members of the Vessel Operator's staff acknowledging that they will abide by the policies and procedures of the OSVDPA.

Partnerships. The OSVDPA utilizes the United States Internal Revenue Service definition of partnership. Specifically, "A partnership is the relationship between two or more persons who join to carry on a trade or business, with each person contributing money, property, labor, or skill and each expecting to share in the profits and losses of the business whether or not a formal partnership agreement is made.".

Principal. A representative of the training provider, vessel operator, or other OSVDPA partnering entity who is authorized to contractually obligate the entity.

Point of Contact. An individual who has intricate knowledge and can answer OSVDPA questions about the training provider, vessel operator, or other OSVDPA partnering entity's proposal, application, request, or submission. For training providers, this individual shall be listed publicly as the point of contact for Prospective DPOs and DPOs. They do not need to have signed a Form P-2-CV, the Participants Rights and Responsibilities Acknowledgement (Current Version) or Form P-3-CV, the Participant Supervisory Rights and Responsibility Acknowledgement (Current Version) or be a Principal of the entity.

Related Entity. The OSVDPA utilizes the United States Internal Revenue Service definition for related entities. This definition, found at 26 United States Code, section 168(h) states that two entities are related if the following apply: "(1) The two entities have (i) significant common purposes and substantial common membership or (ii) directly or indirectly substantial common direction or control; or (2) Either entity owns (directly or through one or more entities) a 50 percent or greater interest in the capital or profits of the other. For this purpose, entities treated as related entities under the preceding paragraph shall be treated as on entity.".

Strategic Relationship Agreement. The agreement defines the nature of the relationship between the OSVDPA and the training provider, sets the responsibilities and includes a nondisclosure agreement.

Qualified on Board Assessor (QOBA). A DPO certified by the OSVDPA, to conduct the Phase 4 and Phase 5 Assessments. In conducting these assessments, the QOBA utilizes criteria established by the OSVDPA to evaluate whether the DPO or Prospective DPO has passed the assessment.

Vessel Operator. A company that owns or operates DP-equipped vessels and employs or contracts DPOs.

15. Forms.

The following forms are referenced in Form VO-1-CV, the Vessel Operator Enrollment Application. A more detailed descriptions of each form can be found in OSVDPA MPP-1-CV.

Form DPO-1-CV, the Training and Certification Activities Confirmation Letter (Current Version. The purpose of Form DPO-1-CV is ensuring that the Vessel Operator or entity that employs the DPO or Prospective DPO confirms and verifies that the DPO or Prospective DPO has completed - the best of the Vessel Operator's knowledge - the Training and Certification Activities. For Enrolled Vessel Operators, the signer of Form DPO-1-CV must be an individual serving as President, Vice President for Personnel/Human Resources, Vice President for Operations, Personnel Director, Marine Superintendent, or Operations Manager or someone who has previously received permission from the OSVDPA to sign this form. Additionally, this individual must have on file at the OSVDPA a signed copy of Form P-2-CV, the Participant Rights and Responsibilities Acknowledgement (Current Version) or the individual's supervisor has signed Form P-3-CV, the Participant Supervisory Rights and Responsibility Acknowledgement (Current Version).

Form P-2-CV, the Participant Rights and Responsibilities Acknowledgement (Current Version). This form must be signed by Vessel Operator employees that sign copies of Form DPO-1-CV. This form requires that individual to acknowledge that they will abide by the policies and procedures of the OSVDPA. Only Principals or those with OSVDPA approvals (i.e., DPO, Auditor, Qualified on Board Assessor) may sign this form (other should see Form P-2-CV below). 

Form P-3-CV, the Participant Supervisory Rights and Responsibility Acknowledgement (Current Version). This form must be signed by supervisors acknowledging that the signer is responsible for the actions of those serving under him or her as these actions relate to the OSVDPA's DPO Certification Scheme. 

Form RE-1-CV, the Related Entity or Partnership Description (Current Version). This form is used by Vessel Operators that have a related entity or partnership with another company. The form acknowledges the relationship, states that neither entity is independent of the other entity (relevant to OSVDPA audits and some types of assessments), and states (at the discretion of the entities involved) that in relation to the OSVDPA DPO Certification Scheme the two parties are represented by one Principal.

For U-1-CV, the Strategic Relationship Agreement (Current Version). This document dictates the terms of the relationship between the OSVDPA and the Vessel Operator.

16. Instructions.

This form shall be completed by Vessel Operators seeking to enroll in the OSVDPA.

All attachments should be clearly and distinctly named, with the names readily apparent on the attachments as the title of the document.

Form VO-1-CV shall be submitted to the OSVDPA as a package with all forms and attachments included.

Once completed, and if being submitted via email, the form and all required attachments shall be sent to ben.berson@osvdpa.org.

Once the OSVDPA receives this form (along with payment of Fee VO-1 (and any other required fees) as listed in OSVDPA FS-1-CV, the OSVDPA Fee Schedule (Current Version)), the OSVDPA shall evaluate the information contained in this form and respond to the Vessel Operator with one of the following responses:

  • Enrolled. If the OSVDPA finds that a Vessel Operator has met all of the requirements, the OSVDPA shall provide the Vessel Operator with a Form VO-2-CV, the Vessel Operator Enrollment Certification (Current Version). This form allows the Vessel Operator to begin conducting OSVDPA Assessments.
  • Provisional Enrollment. If the OSVDPA finds that the Vessel Operator has met the majority of the requirements to be an Enrolled Vessel Operator but has a few small deficiencies, the OSVDPA shall issue a Form VO-2-CV providing Provisional Enrollment, thereby providing all of the rights and responsibilities as full enrollment on the condition that the identified shortfalls be addressed. Such a decision may be accompanied by the requirement that the Vessel Operator pay additional fees, as detailed in OSVDPA FS-1-CV, depending upon the amount of review and instruction that the OSVDPA is required to provide to the Vessel Operator.
  • Not Enrolled. If the OSVDPA finds that a Vessel Operator has significant deficiencies, the OSVDPA shall issue a decision of Not Enrolled. Such a decision shall be accompanied by a specific list of the deficiencies and instructions for addressing these deficiencies
